Abstract:Cardiac arrhythmia is a major cause of morbidity and mortality in heart disease. It has a large impact on the quality of life for a great number of people, and it places a huge economic burden on society. Numerous widely used medications can be a trigger for arrhythmia. Assessment of pro-arrhythmic potential is essential during drug development. Many in vivo, in vitro and in silico models are designed to study cardiac arrhythmias, but there are drawbacks for their use. There is a need for more predictable human-based in vitro models focusing on cardiac arrhythmia. In this study, the arrhythmia shape was designed to obtain a vulnerable substrate for cardiac arrhythmia, by mimicking the source to sink mismatch in cardiac tissue. Arrhythmia-like events were triggered through subjecting the cardiac tissues to Epinephrine. Amiodarone and Quinidine were successful in counteracting the effects of Epinephrine on a monolayer of hiPSC-CMs. Minimal counteracting effects of Amiodarone and Quinidine were found in cardiac tissues cultured in the arrhythmia shapes. Nevertheless, the first steps towards designing a heart-on-chip model to study cardiac arrhythmias were taken. In the near future, this heart-on-chip model could be further developed into a multidisciplinary platform for risk assessment of cardiac arrhythmia.
