Abstract:Trochleoplasty is an operation executed by orthopedic surgeons on patients that present with patellofemoral instability due to trochlear dysplasia. According to these surgeons it is hard to determine what the direction, angle, and depth of the trochlear groove should be to achieve the optimal outcome for a specific patient. To solve this problem a computed tomography scan of a patient with trochlear dysplasia was used to make a three-dimensional model of the knee, on which a simulated trochleoplasty (and two other surgeries, which are usually combined with it in clinical practice) were executed. With this model, the biomechanics of a preoperative and postoperative knee were compared after a simulation with Anybody. As far as the authors know, this is the first time such a combination of surgeries is analyzed. The results of the simulations showed higher patellofemoral stability, but an increased risk of osteoarthritis due to higher patellofemoral contact forces. This study also proves biomechanical simulations are a viable method of assessing the effect of trochleoplastic techniques. If more research is conducted, in the future it could be possible to find a patient-specific optimal shape of the trochlea. This could be the basis of creating a surgical guide.
