Abstract:Introduction: Cerebral autoregulation (CA) allows the brain an adequate blood flow during blood pressure challenges. CA may be impaired in several diseases, but clinical measurement of CA is challenging. Trancranial Doppler (TCD) with blood pressure (BP) measurements are currently the most used technique, but clinical implementation is difficult. Recently, Near-infrared spectroscopy (NIRS) showed a CA estimation similar to TCD/BP. Validation of the NIRS-CA method is needed in clinically relevant conditions. Methods: NIRS- and TCD/BP- derived CA was measured in ischemic stroke patients in the acute phase after intra-arterial thrombectomy. CA-indices were compared between the affected and unaffected hemispheres, and between TCD/BP and NIRS. Results: Nine of 16 measurement had to be excluded due to insufficient data quality. There was a statistically significant difference of NIRS LF phase between affected and unaffected hemispheres. No differences were found for other NIRS and TCD/BP CA-indices. NIRS and TCD/BP LF phase were significantly correlated, but the absolute values were different. Discussion and conclusion: Measurement setup optimisation are discussed. Interpretation of the NIRS and TCD/BP data should focus on other relevant parameters, in addition to the CA indices. A comparison of clinical outcome with NIRS parameters should be performed when more data is available.
