Abstract:Sarcoidosis is a multisystem inflammatory disorder primarily represented in the lungs and the mediastinal lymph nodes. The current diagnosis is performed via endobronchial ultrasound assisted transbronchial needle aspiration (EBUS-TBNA), which has a limited negative predictive value. A potential technique to solve this problem is contrast enhanced ultrasound (CEUS). The goal of this study was to find out if CEUS could provide the information needed for improvement of the diagnosis of sarcoidosis and could fit in the current clinical workflow. A review of clinical and technical literature was performed by searching the databases Scopus, FindUT and Google Scholar. Practical in-vivo research with CEUS was performed in the pancreas of a female patient. A hypothesis was created on the manifestation of sarcoid lymph nodes in the derived parameters from the quantification of CEUS. A protocol for a CEUS procedure was created and an insight into an ideal diagnostic process for sarcoidosis, a short review on alternative techniques and recommendations for additional research were given.CEUS could provide added benefit on different aspects in the diagnosis of sarcoidosis in the mediastinal lymph nodes, considering quantification of the enhancement, the ease of implementation and the potential to stage and influence the prognosis and treatment.
