Abstract:The objective of this research is to find out which determinants have a facilitating or impeding influence on the implementation process of the renewed questionnaire developed by GGD Twente to support the contact moment between adolescents and Child Health Care (CHC) professionals at high schools, according to the opinions of the stakeholders. The research design is explorative with qualitative data collection methods. By interviewing 6 adolescents and 4 mentors and organizing an online focus group for 4 CHC professionals the opinions of the stakeholders were collected. It can be concluded that the most important facilitators are associated with the innovation itself, which is overall an improvement compared to the former questionnaire. Adolescents do like the questionnaire and are willing to cooperate. However, the questionnaire should be more effective if the adolescents should fill in the questionnaire in a situation wherein the adolescents sit separately, it is quiet and every adolescent stays for a certain time period. Mentors should be more aware of the procedures around the questionnaire. A protocol should be made for the professionals about the procedures after they receive the completed questionnaires.
