Abstract:Background: Posttraumatic stress disorder (PTSD) with a prevalence of 1 till 14 percent in the normal population and 3 till 58 percent in the group of at-risk individuals is not a rare disorder and sleep disorders are a common symptom. 70- 91 percent of patients with PTSD suffer from insomnia, 19-71 percent suffer from nightmares and the resulting fatigue. The kind of relationship between fatigue and PTSD, with regard to etiology, maintenance and treatment, remains unclear. For this reason, literature regarding this subject is targeted to explore the connections between PTSD and fatigue, using systematic review methods. Methods: The literature search was conducted in the spring of 2016. In order to gain a broad understanding of the subject and due to the large definition for fatigue, not only fatigue is used but also synonyms, as well as related concepts such as exhaustion, sleepiness and sleep quality. Only published literature that contains information on PTSD, fatigue, etiology, maintenance, psycho- and pharmacotherapy is used. Results: It has been found that a volume change of the gray matter, and the hippocampus is associated with sleep problems. Further, an increase in the neurotransmitter nitric oxide / peroxynitrite is found in the brain after a traumatic event. These increases correlate with the development of PTSD symptoms including fatigue. Decreased cortisol level are found in PTSD patients, which is also found in patients with fatigue symptoms. Furthermore, it is found that there is an interaction between poor sleep and the emotions after a threatening event. This interaction effects the emotion, sleep duration and sleep quality of the patients and sleep problems are associated with the development of PTSD. When turned to psychotherapy, IRT combined with CGT- I, EMDR and physiologic sport therapy give good results. For Pharmacotherapy is pavozine hopeful because the drug also reduced both insomnia and nightmares. Discussion / Conclusion: This literature review can conclude that the relationship between PTSD and fatigue is now not enough studied because most studies focus only on sleeping not fatigue. However, there is evidence found for further studies for correlations of sleep and PTSD as well as for correlations of fatigue and PTSD.
