Abstract:Background: Due to recent developments in the field of psychology, mental health is no longer defined by only the absence of disease, but also by the presence of well-being. The two continua model of mental health approaches positive mental health and mental disease as two related but different continua. Although there is sufficient evidence for this model and the three factor structure of well-being in the general population, support is lacking in a clinical population. In this study the three factor structure of well-being and the two continua model are validated in a clinical population. Method: Routine Outcome Monitoring (ROM) data was used from the intake of 498 ambulant adult clients in the Dutch basic and specialistic mental health care. The ROM consisted of the MHC-SF as measure of well-being and the OQ-45 as measure of mental illness. The structure of both the MHC-SF and the two continua model were tested with exploratory and confirmatory factor analysis. Results: Both the exploratory and confirmatory factor analysis of the MHC-SF did not unambiguously show the expected structure of well-being in the MHC-SF. In the confirmatory analysis, the two continua model did provide a good fit of the data and a better fit than other tested models. The exploratory analysis however yielded only a single factor. Post-hoc exploratory factor analysis with the group splitted based on whether the score on the OQ-45 was above or below the cut-off point for psychological disfunctioning, showed two related factors for the group below the cut-off, in contrast to the full sample. Conclusion: The three factor structure of well-being could not unambiguously be confirmed in this study. The two continua model can be confirmed, however the relation between psychopathology and well-being is stronger compared to within the general population. The validation of the two continua model in a clinical population confirms the need for integrated mental healthcare, with focus on both psychopathology and well-being.
