Abstract:Since 2014 colorectal carcinoma (CRC) became the second most common cancer in the Netherlands. Although classification according to TNM and UICC stage provides prognostic information and guides towards therapy decisions, the therapy response and outcome of individual patients is not predicted. In recent years the proliferation marker Ki-67 has proven to be a prognostic marker for colorectal cancer. However, current methods are still dependent of visual estimation and/or manual counting by the pathologist. Results are subjective and lead to high intra- and interobserver variability. For this study the objective is to determine the Ki-67 index more accurately with an automatic quantification tool. Tissue sections were prepared from surgically resected colorectal carcinomas in patients who went for surgery during the period of 1996-2006 at Medisch Spectrum Twente (Enschede, The Netherlands). The tissue sections of the primary tumour were dissected into 2µm sections and fixated onto microscopy slides. The epithelial marker CAM 5.2 stains the tumour epithelial area, which through co-registration will be overlayed on the Ki-67 stained tissue to automatically detect the correct nuclei. The automatic quantification tool calculated and the two pathologists estimated the Ki-67 index in 30 patient samples. A novel assessment for the Ki-67 proliferation was introduced.
