Abstract:The role of eHealth interventions in mental health care has increased significantly over the past few years. Despite many forms of criticism on eHealth, many studies nowadays show that online therapies can provide good treatment results. The use of an underlying theory appears to have an influence on the success of the intervention. The extent to which therapists work in accordance with this theory also appears to be a predictor of success. This phenomenon is referred to as therapy consistency. In this study the degree of therapy consistency of the e-mailcounsellors of the online ACT-intervention ‘Living with Pain’ was measured by means of a specific coding-scheme. Results were then compared to the treatment outcome of the intervention which can be reflected in a significant improvement in the level of psychological flexibility in chronic pain patients. Descriptive statistics show that every counsellor achieved a mean ACT-consistency score between 0 and 1. The subsequent t-tests prove that there were significant differences in ACT-consistency for each individual counsellor. Finally several ANOVA measurements show that no differences were found in the way counsellors differed from each other in ACT-consistency. Also no differences were found between participants and the extent in which they differed from each other in the post hoc improvement in psychological flexibility. Results show that all counselors were able to respond ACT-consistent to some extent. However, they were unable to guarantee continuity in ACT-consistency to individual patients. Furthermore it appears that ACT-consistency does not relate to psychological flexibility. The findings suggest that ACT-consistency was not fully embedded in the intervention, possibly due to several limiting aspects of e-mailcounselling compared to face-to-face therapy. Furthermore the question arises whether or not solely a coding scheme for sentence ratings is sufficient enough to fully measure ACT-consistency. Finally, the proven lack of continuity suggests that, due to causes further to be investigated, counsellors were not adequate enough to act in an ACT-consistent manner.
