Abstract:Epilepsy is one of the main causes of brain health disability. In 70% of the patients, seizure freedom is provided with medication. Patients with focal epilepsy, who have not responded to intensive medical treatment, are screened for the possibility of epilepsy surgery. In more complicated situations, when no visible lesion on an MRI image is found, or the lesion overlaps with unctional areas, electrocorticography (ECoG) is performed with chronic grid implantation for maximally ten days. During this long-term ECoG recording, the seizure onset zone (SOZ) is delineated based on the seizures occurring during the chronic ECoG monitoring, and the resection area is determined based on the SOZ. With single pulse electrical stimulation (SPES), the SOZ can be determined without waiting for a seizure. In this study, we have developed an automatic detection algorithm to determine the location of the SOZ and we investigated how to improve the SPES protocol towards a more time-efficient protocol.
