Abstract:Introduction. The antibiotic resistance increases for all bacteria in The Netherlands, despite the low level of antibiotic resistance in the country compared to the other countries in the European Union. Antibiotic resistance means that bacteria develop insensitivities for antimicrobial agents, which is caused through inappropriate applying of the antibiotic agents. Because of this resistance, the antimicrobial agent is no longer able to kill the bacterium or inhibit the growth. Antibiotic resistance is caused by non optimal use of antimicrobial agents. In a response to these matters the Antibiotic Stewardship Program (ASP) is developed, this is seen as an essential for the Dutch hosiptals to have a successful antibiotic policy. To labour on the reduction of antibiotic resistance eHealth is used, with eLearning as one of the applications. To strengthen the attitude and behavior changes of the healthcare professionals, eLearning makes use of persuasive strategies. This study researched what the current situation is for eLearning, which content aspects about antimicrobial agents should be integrated in the eLearning course and where eLearning courses have to match antimicrobial agents goals for the healthcare professionals of the University Medical Center Groningen (UMCG). Method. This research contains two stages of the Centre for eHealth Research and Disease Mangement (CeHRes) Roadmap; stage 1 ‘Contextual Inquiry’ and stage 2 ‘Value Specification’. The ‘Contextual Inquiry’ identifies the potential stakeholders and work processes in regards to antimicrobial agents through observations with eight participants and one interview. The stakeholders are participants that are important in the research. In addition, the current development of eLearning is analyzed. The ‘Value Specification’ added important values for the development of the eLearning course, recruited among stakeholders through 14 individual interviews with several healthcare professionals and one eLearning expert. In these interviews different materials have been used: the theme scenarios about specific aspects of antimicrobials, the consisting eLearning courses on antimicrobial agents and a mock up of possible eLearning course. Results. The current antimicrobial agents situation can be improved in the UMCG. Because too often the antimicrobial agents are too wide spread, too frequently and for a extended period of time being prescribed and used. This situation can be optimized by working with an eLearning course for healthcare professionals. By increasing knowledge about antimicrobial agents, healthcare professionals can develop more awareness for dealing with antimicrobial agents. The healthcare professionals can optimize antibiotic use, a result might be safer and more responsible use of antibiotics. This allows the patient to go home earlier, less development of antibiotic resistence and there may be lower costs. The roles of the healthcare professionals in the UMCG in regards to antimicrobial agents are wide spread. The physicians decide to prescribe antimicrobial agents, the nurses prepare and serve the antibiotics to the patient, and the experts from the department of Medical Microbiology support the healthcare professionals. The physicians and nurses can be more intensive supported in the information supply of antimicrobial agents, which they can apply in their function. The (persuasive) elements which the healthcare professionals find important in an eLearning course, are the duration of the eLearning course, validation of the results of the eLearning course, the practical functionality of information about the content and the option to make contact with the ICT department and the department of Medical Microbiology for any questions or when the information is not clear. The important functionalities by making the eLearning course according the stakeholders are: • The eLearning course should be focused on the practical situation of healthcare professionals. • The eLearning course must have a learning curve that goes from an easy to a more difficult level, and from general to specific knowledge of antimicrobial agents. • A combination of the use of media that is composed of video, text, audio and picture. This increases the effectivity of learning and it is attractive perceived by the stakeholders. • The final knowledge test must also provide adequate feedback. The healthcare professionals experience the tools, that provide a support in making the eLearning course very positive, for example the antibiotic application. The healthcare professionals indicate that the application of antibiotics both in real-time situation and in the completion of the eLearning course could be effective, because it has background information on antimicrobial agents. The healthcare professionals find themselves responsible to keep up to date on recent knowledge and skills in their trades field. As a result of the previous, some healthcare professionals react reserved on an obliged eLearning course. An obliged eLearning course could also encourage cheating. The degree of a successful implementation of the eLearning course depends on a number of factors, such as the system and the content. A motivation to successfully complete the eLearning course is to receive personal accreditation points. Some healthcare professionals in the UMCG are required to obtain accreditation points to maintain their professional skills. Accreditation will guarantee the reliaibility and quality of the eLearning course. The accreditation organization for nurses is the ‘Kwaliteitsregister Verpleegkundigen en Verzorgenden’, and for physicians and specialists the ‘Gemeenschappelijke Accreditatie Internet Applicatie (GAIA)’. These organizations have specific requirements to develop an accredited eLearning course, with elements covering learning objectives, content and didactic criteria. Conclusion. The stakeholders of the UMCG regarding antimicrobials are: the physicians (decide), nurses (preparation and actual treatment of the patiënt) and the department of Medical Microbiology (support the healthcare professionals). The current situation in the UMCG according to antimicrobial agents can be improved and optimized by saver and more responsible use of antibiotic agents. Furthermore, an interactive eLearning course can be a valuable tool to furfill the information needs, increase knowledge, keep knowledge up to date, and create and maintain awareness of antimicrobial agents for the nurses and physicians. The content, functionalities and (persuasive) elements are of great importance for a successful implementation, because they should connect to the stakeholders, particularly the final users. Tools with a provision of information may be used to support the healthcare professionals in the use of antimicrobial agents in the eLearning course and in practical situations, such as the tool application antibiotics. The extent to which the eLearning course is completed successfully depends on several factors, including the system and the content. To what extent the eLearning course is successfully completed is the responsibility of the healthcare professional. The eLearning course can be made reliable and attractive by accreditation, whereby healthcare professionals can obtain personal accreditation points. A successful implementation of an eLearning course in the UMCG is depending on a high level support within the hospital and the level of involvement of the healthcare professionals.
