Abstract:Background: Healthcare is increasingly confronted with threats caused by healthcare-associated infections. Public Health services are in demand to deal with the problem. Care displaces more often from hospitals to nursing homes and as a result nursing homes have to deal more frequently with these infections. The EurSafety Health-net project deals within the Netherlands and Germany among other things with prevalence measures of healthcare-associated infections. Within this project an eHealth application is developed and implemented in the Netherlands. In this study, the focus was set on the development of a German version of this application and to find out which requirements are needed to implement the application in German nursing homes. Objective: Aim of this study was to get insight in how the Dutch application has to be changed to fit in the German Healthcare system. For this purpose, stakeholders within the German nursing home context were asked about their expectations, needs and wishes about the application and the current situation is inventoried. Methods: The study was conducted in four phases. During the first one a literature study is done to get an overview of the current situation in nursing homes and the differences between the healthcare systems. In the second phase, Interviews with nursing home manager en hygiene specialists were done to see the current situations and find out how the application can fit the stakeholders needs. The third phase was the development of the prototype van de application. This prototype was tested within phase four with a think-aloud usability testing with paper-prototyping elements. Results: The results have shown that the nursing homes reached different stadiums with regard to infrastructural readiness for change and that an implementation has to be planned step by step so that it fits for every nursing home. The application has to be changed with regard to the content because of the wishes of the stakeholders. The design en performance can stay the same as in the Dutch version. Conclusion: To implement the eHealth application in the German healthcare system the current situation of the nursing homes has to be inventoried in detail. The stadiums seem to be an important key factor for the willingness and motivation to use the application.
