Abstract:Background: Within the pulmonary department of MST there is already done years of researching COPD patients. For this is, a COPD cohort started in 2005 with 795 COPD patients. It was primary to mortality and inflammation. Adherence is an important area of research in the cohort. This adherence is determined using pharmacy data with a follow up of 3 years and is divided into optimal, sub-optimal, under-and over-use. Adherence is an important theme in self-management of chronic physical illness like COPD. In this study are the determinants and underlying motives related to over-use and under-use of inhalation medication obsolete. Methods: Design: The design of this research consisted semi-structured in-depth interviews running with COPD patients. Then on the basis of a thematic analysis of the obtained data the research questions answered. Respondents: respondents who participated in the study were from the COMIC cohort of Koehorst-ter Huurne, Movig, Van der Valk, & Varma (2015). Procedure: For the study, patients were first contacted by telephone. Then they were given an information letter. If the patient agreed to the request to participate in the study, the patient was asked to send the form back. During the actual interview, the patient was again informed about the study, the patient was given space to ask questions and voluntary basis was emphasized. Afterwards, the respondent was again given the opportunity to ask questions. Measurements: The questions focused on the personal dependent factors, treatment factors and pharmacological factors that possibly influenced the compliance of patients with COPD. Results: From the results obtained from the analysis of the study shows that there are no major differences in the category personal dependent factors. In the theme knowledge and perception are the relatively largest differences in the degree of obstruction by COPD and the given rating for the severity of the disease COPD. The overusers experienced on average more barriers and often give a higher rating. The overusers also have a little more confidence in the treatment and course of COPD. Also exhibit overusers something more depressive symptoms. In the treatment factors are no big differences. The overusers undergo slightly more treatments. Concerning the medicine factors, there is also little difference. The overusers use on average slightly more medications for COPD and other conditions. Conclusion: between the underusers and overusers is little difference. It turns out that researched determinants to non-adherence can lead, however, that in the form of both underuse as overuse. However, there can be seen striking patterns. It is noticeable that respondents have little knowledge about the disease and treatment, relatively high depressive symptoms, are more interested in the short-term than in the long-term perspective, they may use coping strategies and trends in the results show that smoking may lead to compensation behavior which in turn may lead to over-use.
