Abstract:Background: Despite the proven effectiveness of web-based treatment for mental disorders, there are still many questions about how to development of a solid therapeutic alliance between counselor and client. Objective: To gain insight in the assessment of the therapeutic alliance by healthcare workers and clients within the online treatment Etendebaas.nl. As well as obtaining an understanding of the relationship between therapeutic alliance and the treatment effects and drop out. Methods: The ' Helping Alliance Questionnaire ' (HAQ) is assessed from clients as well as healthcare workers at the time of the online Etendebaas.nl intervention (N = 200). The score on the client and counselor version is among themselves, as well as, on different time points compared with each other within a Randomized Controlled Trail (RCT). Using paired t-test, Pearson correlation analyses and Bland & Altman analysis is the degree of similarity in assessment of the therapeutic alliance measured between healthcare workers and clients. In addition, Pearson correlation analysis and linear regression analyses used to investigate the therapeutic alliance as a predictor of treatment effect. Results: This study shows that both healthcare workers and clients consider the therapeutic alliance in online treatment as positive. With both start- and end scores of the therapeutic alliance as reviewed by client (M=42.7, M=45.5) and counselor (M=41.8, M=43.8). Clients rated the therapeutic alliance at drop-out (M=33.4) as significant worse than healthcare workers (M=36.4). The client and counselor rating on the intervention period correlated significantly weak with the treatment outcome. It turns out that the assessment of the therapeutic alliance of clients on the intervention period, a significant predictor (15%) is for the treatment effect, as well as the opinion of clients drop out (17%). The judgment of the healthcare worker doesn’t show any predictive value for treatment effect. Conclusion: Good therapeutic alliance can be developed during the investigated online treatment. The alliance developing turned out positive during the treatment, by both client and therapist, however only a positive experience of the client showed to add predictive value to the treatment effect. It is unclear, however, whether the therapeutic alliance has a direct effect on the treatment effect or that this relationship is mediated by drop-out. More research is needed to identify the active components of online treatment interventions and which elements are responsible for the treatment effect and the therapeutic alliance.
