Abstract:Introduction Due to an increasing need for self-management in care for patients with Type II Diabetes Mellitus (T2DM) this study proposes a new pathway with support of a Personal Health Record (PHR) such as e-Vita. Methods Using a literature review we attempted to establish a pathway fitted to the use of a PHR. A search is carried out for the added value of a PHR, the demands of patients and health care providers and the possible complications associated with implementation. Results Literature shows that PHRs have beneficial effects related to clinical outcomes and quality of life due to the improvement in self-management. Education, communication and user-friendliness are important factors in this improvement. Discussion The new pathway cannot be used as general guideline for T2DM because of variety in general practices in the Netherlands and the platform e-Vita is not representative for all PHRs. Further research is needed regarding the various factors that affect the usability of a PHR, the influences of various components of a PHR on self-management and the demands and requirements of health care providers about the use of a PHR. Conclusion A Personal Health Record can be an added value through preparation prior to consultations, education provision, tracking metrics, setting goals and interaction between the health care provider and the patient.
