Abstract:Research question “What is the primary process of planning and execution of PET-CT scans and how can the resulting workload at the administration office be reduced without increasing acces time or using more resources” Setting Nuclear medicine department of Isala hospital Zwolle, The Netherlands. Methods Observational study using flow charts and value chain to map processes and stopwatch time studies and analysis of planning programme output in order to gain quantified data for each process. Results and conclusions Workload is increased by three main causes: missing data in request forms for scans, unclear division of labour at the administration office and extra work due to the way in which the planning process takes place. Short-term recommendations Implementation of a standard point of quality control of request forms before planning takes place will reduce amount of time spent on planning. in order to reduce amount of missing data, incomplete forms should be registered and given as feedback to requesting doctor. To improve division of labour, secretaries should redesign the workings of the administration office under guidance of lean-specialists. Work should be standardized and documented. Change of duration of scan by doctor after appointment is already made will disturb planning. Do not plan appointments before check by doctor unless sure that no change will take place. Planning all appointments needed for scan as combination-appointments as much as possible will save time. Give patients appointment information by phone while simultaneously going to checklist instead of contacting patient twice. Send reminder through text or e-mail. Use blocks of one hour to block time for emergency patients or other forms of block-planning. Long-term recommendations The requesting doctor is the primary doctor for a patient. The doctor should act as primary contact for the patient and prepare the patient for the scan. In the longer term, a patient can have one contact for all hospital-related business. Indicators can be used to monitor progress made at improving quality control over longer periods of time.
