Abstract:Introduction: Alcoholdebaas.nl (AdB) is a web-based treatment guided by a counselor. AdB has been proven to be effective in reducing the use of alcohol, but the role of counselor communication is unknown. Motivational interviewing (MG), the therapeutic alliance (TR) and other skills (OCV), like empathy, have proven to be important for the course and outcome of a treatment. The relation between the quality of communication techniques of MG, TR and OCV and dropout and success of treatment was examined. Success of treatment was determined if the client met the norm of low-risk drinking. It was expected that a higher quality of communication of MG, TR and OCV resulted in lower dropout (hypothesis 1) and a higher amount of successful treatments. Finally, it was examined if a quick subjective way of judging quality of communication matches the judgments given by an objective way. Method: A score sheet has been used to assess the quality of communication. Communication techniques of MG, TR and OCV were scored on a 3-point Likert scale (insufficient, sufficient and good). Based on these scores, total scores for MG, TR and OCV were calculated. Per analysis a quick subjective score had to be given that represents the overall impression regarding the quality of communication. This score had to be given independent of the scores given with the score sheet. The scores given by the score sheet were coupled to an existing data file of AdB, which contained data about dropout and successful treatments. Results: Quality of communication for all treatments was found best for MG and OCV. Quality of communication was scored primarily as ‘’sufficient’’ for TR. Quality of communication was found better for the communication techniques ‘’resistance’’ (MG), the total score of MG and ‘’empathy’’ (OCV) for clients who have completed the whole treatment (completers). The total score of MG also predicted a lower change of dropout. Almost significant differences in quality of communication were found for the communication technique ‘’support and endorsement’’ (MG), where the quality was found better for completers. No significant differences were found for successful treatments, but an almost significant better quality of communication was found for ‘’confrontation’’ (MG), the total score of MG and ‘’feedback’’ (OCV) for clients who completed the treatment unsuccessful. An almost significant better quality for ‘’reflecting on objectives’’ (TR) was found for clients who completed the treatment successfully. The objective scores of MG and OCV showed a significant positive correlation with the quick subjective way of scoring. A non-significant positive correlation was found between the objective scores of TR and the quick subjective score. Conclusion: Results show that the quality of communication differs significantly for dropouts and completers concerning some communication techniques. Also, more insight has been obtained about the use of communication techniques and the relation with dropout and success of treatment. The quick and subjective way of scoring looks to be an alternative (compared to the objective way) if the quality of communication has to be judged in a short amount of time. Because of the scarcity of research about this subject, more research is necessary.
