Abstract:Objective. The perspective of patients with rheumatic diseases is receiving an increasing importance in the evaluation of treatments. There are often symptom specific questionnaires in use for evaluation, which are completed by the patients. Based on the completed questionnaires, a change in the measured symptom can be detected. The purpose of this study is to exploratively compare the similarities between the measured changes shown by the questionnaires and the subjectively perceived changes by the patient. Method. 330 respondents with polyarthritis (60.9% women, mean age: 61.55 years) completed questionnaires regarding the following aspects: pain, fatigue, depression, purpose in life, positive affect and the satisfaction with participation. The questionnaires were completed at three different moments in time. During the second and third measurement, there were questions included relating to the perceived changes of the aspects by the patient. The distributions of the difference scores of the questionnaires and the scores of the questions with regard to the perceived changes were joined together by means of crosstabs. Cohen's Kappa and Spearman's rho were calculated for each crosstab. Conclusion. The results showed that the two methods of measurement need to be regarded as independent. The pattern in the crosstabs for the physical aspects showed that the question related to the perceived change is more sensitive for measuring change than the difference scores. Implications. Further research should look for the reasons for the mismatch between the two methods. This will improve both methods and benefit both the physician and the patient when evaluating treatments.
