Abstract:Background Among the heavy drinkers three percent develops Korsakoff syndrome, an estimation of 8.000 to 10.000 people in the Netherlands. The syndrome is characterized by memory- and executive disorders. It is most common in chronic alcoholics but it is caused by a serious deficiency of thiamine. The memory disorders are not restorable and because of that there is a need of counseling in big- and small-scale institutions. Small-scale living is defined as a group with six to eight residents who form an household and live according to family model. Research has shown that caretakers are more positive about small-scale living than about large-scale departments. Moreover, it appears that younger, unexperienced caretakers have more trouble with the independency of small-scale living. In this research there will be a focus on work experience. The research question is: What are the differences in experience with small-scale living for Korsakoff patients of caretakers at ZorgAccent with less versus more work experience. Methods Written surveys were conducted with 61 caretakers who work at ZorgAccent. Of the 61 caretakers 59 caretakers filled in the survey. The survey consisted of a questionnaire about job satisfaction, work motivation and open questions about advantages, disadvantages, skills, knowledge, tasks and flexibility of small-scale living. After the survey was conducted there were also eight interviews. This interviews consisted of the open questions from the survey. The questionnaire was analyzed with an one-way ANOVA. The interviews and the open questions were transcribed and divided in fragments, which were labeled accordingly. This led to a table with core- and subcodes. Results Caretakers with one to five year work experience are less satisfied with the quality of the care, the contact with the patient and the total score of job satisfaction than caretakers with more work experience. There were no significant results on work motivation at different years of work experience from caretakers. It appears that caretakers are more positive about small-scale living. The advantages they named are domesticity, more individual attention and security. The caretakers see working alone and the threatening feeling for the residents as a disadvantage. Some caretakers do not notice any difference with skills and knowledge while other caretakers do notice differences. Furthermore caretakers find the tasks of small-scale living not difficult, but different. Finally, caretakers say that they have no problems or see no differences in the flexibility of small-scale living. Discussion In this research it should be kept in mind that the four groups of caretakers are disproportional as regarding to group size. For more reliable statements the group sizes should be equal. Also there are comparisons with literature from which the research is focused on a comparison between large-scale and small-scale settings with residents who have dementia. There is no comparison between caretakers with less versus more work experience with residents who have the syndrome of Korsakoff.
