Abstract:Background. The life-review course ‘Op verhaal komen’, was reviewed in an impact study in 2011 and shown to be effective. The course focuses on writing about life; participants weekly write assignments about different times in their lives and get feedback by their councelor. The correspondence takes place through email. The impact study showed improved psychological well-being and reduced repressive and general symptoms. This narrative study focuses on non-adherence; the drop-outs. Goal of this study is to find predictors of non-adherence in the correspondence between the councelor and participant during the course. In addition it is checked if there is a correlation between non-adherence and the construction of identities during the course. Methods. The course ‘Op verhaal komen’ had 58 participants, of which 14 didn’t complete the course. Three of the 58 cases were assigned to this study for further investigation, based on their (non)adherence. A criteria was that they finished up to week4 of the course. After the analyses were finished, the (non)adherence of the three cases was revealed to the researcher, this study was carried out blindly. The analysis of the correspondence was carried out through an overall impression of the case and an analysis of the construction of identities. The overall impression focused on factors like motivation and the ease to write about own feelings and thoughts. The construction of identities were analysed by deviding the e-mails into fragments, based on different identities. The fragments got coded by the identities and way of interaction. This study focused on finding correlations between de construction of identities and non-adherence by comparing the three cases. Results. The overall impression as well as the construction of identities showed several predictors of non-adherence. The analysis of identities showed the importance of the construction of the undesirable self, the recovering self and the adherence self. In addition a diversity of different identities shows openness and willingness to the course. The overall impression showed the importance of motivation, and thereby expectations, for the course. Also the ease with which the participant writes about his or her feelings showed significant importance in predicting non-adherence. This can best be detected by analyzing style of writing and overall openness during the course. Conclusion. This narrative study provides insight in the prediction of non-adherence. The results have shown correlation between reasons for non-adherence known from the research literature and elements in the correspondence between councelor and participant. Several factors have shown to be of value in predicting non-adherence. This study confirmed the importance of the analysis of identityconstruction and an overall impression in finding predictors. This study provides a broad perspective on online therapyprocesses and predictors for non-adherence by its unique way of combining different views of the correspondence between a councelor and participant. To validate these findings more research is to be done.
