Abstract:In the Netherlands, the number of nursing homes adds up to 1700 with almost 150.000 eldery living in these homes and being dependent on the help of caregivers. Within this group of elderly 75% have physical impairments and need another person to help them with their daily living and care. Apart from the caregiving, the elderly living in these nursing homes are also dependent on caregivers for their social contact. The relationship between the caregivers and the elderly plays an important role in identifying and fulfilling the wishes and psychological needs of the residents. The mental health of an individual can be enhanced by stimulating the psychological wellbeing, consisting of three basic psychological needs; autonomy, relatedness and competence. Previous studies answered the question how communication aspects, which do fulfill the need for relatedness and autonomy, are used in health care so that residents of nursing homes can fulfill their needs and achieve an higher level of wellbeing. This study will contribute to this topic by investigating the relation between autonomy and relatedness and by inserting the care evaluation of the resident. Through a literature study and conversation analysis, the video fragments of the morning care of one resident and three caregivers have been analyzed. The fact that there are three caregivers and one resident gives us the opportunity to observe the effect of different care relationships and the impact upon care evaluation. The results show that different aspects of relatedness and autonomy contribute to a higher level of psychological wellbeing. There are also factors from the literature study that didn’t provide the expected result for this resident. It has shown that the need of autonomy has a different outcome, the resident stands up for himself when he doens’t agree and that’s where he is autonomous. A framework has been developed for caregivers which include the most important behavioral characteristics for the most optimal need support of relatedness and autonomy. The results also show that the simultaneous support of autonomy and relatedness gives the best results and provides a higher level of psychological wellbeing. The support of autonomy, being the independence and freedom of choice, lays the foundation of a social relationship between the caregiver and the resident, resulting in a positive influence on psychological wellbeing. This result showed that, even if there was need support of relatedness and autonomy, the positive reaction of the caregiver was decisive for the actual experience of need support. For further research there are some recommendations. It’s important to generate a larger group of residents and caregivers, with more diversity and different characteristics. It’s also essential to study the basis psychological need of competence and finally, experimental research of the framework can investigate the influence of this framework on the care relationship between the caregiver and the resident and on the psychological wellbeing of the residents.
