Abstract:Background: The aim of this study is to investigate the needs of breast cancer patients and health care professionals for a breast reconstruction decision aid. Questions were asked about the content (physical outcomes; mental, daily and physical consequences; risks; the procedure; and not having a reconstruction); the presentation of images (photographs or animations), risk information (bar graph, flow chart or 100 faces diagram) and values clarification (personal exercise, patient stories and forum); and implementation (timing, location and computer use). Methods: Patients (N=15) and health care professionals (N=6) were interviewed using an interview schedule with questions about the usefulness and (dis)advantages of a decision aid and the required content, presentation and implementation. In order to gain insight about presentation preferences, respondents were shown mockups with different options. The interviews were audio-recorded, transcribed and after that, analyzed using a coding system. Results: In general, the respondents were positive about the decision aid. Two main results emerged from analyzing the preferred decision aid content. First, patients stated that the option of breast reconstruction could help them create a future perspective. Second, more information was needed about risks of implants. The following presentation was preferred: for showing pictures the use of photographs, for presenting risk-information the use of the flow chart and for clarifying values the use of a personal exercise. There were no apparent implementation preferences, these were different for every individual. Discussion and conclusion: The decision aid can be a complementary tool for the current breast reconstruction education and can be improved by broadening the target population. The future perspective and current issues about implant risks should be discussed. Presentation of various photographs is recommended in order to provide realistic expectations. Risk information should be presented in an understandable way and the decision aid can provide extra clarification in addition to the health professionals’ information. Patient stories and a forum are not recommended because they do not seem to be applicable to this target population. Health care professionals can adjust the implementation strategy to every single patient and therefore play a crucial role. It is recommended to develop the decision aid and conduct further research on the decision aid efficacy
