Abstract:The care for the elderly has been negative in the news for quite some times in the last years. There are more and more elderly people who need care, but there is less and less money and an excessive work overload in care. These factors make it difficult to focus on the needs of the individual. It appears that the contact between caregivers and residents plays an important role in identifying and fulfilling those needs and as a result may lead to higher state of mental wellbeing. However it is unclear how the needs of the elderly are met through communication. The aim of this study is therefore to investigate how the communication between caregivers and nursing home residents during morning care can contribute to higher levels of mental wellbeing. The psychological wellbeing can be stimulated by fulfilling the three basic psychological needs: autonomy, competence and relatedness. This study attempted to answer the question how communication aspects, which fulfill the need of autonomy, are used in health care so that residents of nursing homes can fulfill their need for autonomy and can achieve a higher level of wellbeing. By a literature study and conversation analysis have several video fragments of the morning care been analyzed. This study shows that there are three communication structures that can fulfill the need for autonomy. It appears that when the resident have participation, the fulfillment of autonomy is increased. There are different ways to do this; the resident chooses, the resident has a say, but is influenced by the opinion of the caregiver and the resident has a say after a closed question of the caregiver. A second aspect that is found is that the resident can do what he or she is still able to do. Within this aspect there are two variations. The first is that the resident does it by his or herself. The second is that te caregivers stimulates the independences of the resident by stimulating him or her to do small tasks. Finally it was found that when the caregiver told the resident what will happen, the resident experienced clarity and had a feeling he or she had a say in it. The resident could be informed in what will happen by both verbal or non-verbal communication. Remarkably, the stimulation of autonomy can be done by the resident and caregiver. By example, the resident can take the lead by doing a care task by him or herself. Or he or she can give her opinion before the caregiver ask him or her for their opinion. The caregiver can stimulate the autonomy by giving choices to the resident or by telling what will happen, but also by the residents to perform actions that they can do independently. For further research there are some recommendations; generate a larger group of respondents, do not only use video fragments of the morning care, do not focus on one basic need but on all three basic needs. The results will be much more generalizable. Also it is important to use questionnaires to measure the extent to which the resident says that the communication has been fulfilling. Recommendations regarding the daily practice are using a video-interaction guidance and training of the caregivers in fulfilling the basic needs.
