Abstract:The objective of this systematic review is to investigate the impact of tangible rewards on substance use of young people between 16 and 25 years old. A total of 10 randomized controlled trials and 11 observation studies were included. All 21 studies showed efficacy on the short term on behavior change. Only six of them explored both short and long term effect (> six months). Of these, three randomized controlled trials didn’t prove effect after six months. However, three observation studies (Quit and Win) showed a positive effect after 6 and 12 months, with an average abstinence rate of 23,9% (intention-to-treat: 9,3%), although these results were based on self-report. To investigate the impact of the variables on the effectivity of incentive-based interventions the studies were analyzed for: 1) choice of target behavior, 2) monitoring of target behavior, 3) incentive scheme, 4) size and 5) type of incentive. Five studies showed more effectivity when the target behavior has a direct relationship with the healthy behavior. No evidence was justified by the other variables. Incentive schemes were both effective when the incentive was provided immediate after the desired behavior as well as when there was only a chance to win a prize. One study showed more efficacy when the size of the incentive was higher, but also small incentives were effective. Psychosocial therapy, particularly Motivational Enhancement Therapy, was more effective when implemented with an incentive-based intervention. There was no evidence of other kinds of interventions in combination with incentive-based interventions. We can conclude that incentive-based interventions for young adults can change behavior in the first instance, but to maintain this behavior it’s necessary to develop a more sophisticated programme. Future evaluation research should prioritise the use of randomized controlled trials, as this approach is more reliable than observation studies. This research should concern both short and long term effects as well as the separate impact of the five variables. Moreover all evaluations should be accompanied by well designed process evaluations, to get detailed information about the implementation and the acceptance of the intervention. This might result in a deeper insight in the efficacy of incentive based interventions by young adults.
